Below are screening tools which can help give us a beginning point for your treatment. We may revisit these screening tools periodically to measure your progress and make adjustments to treatment as necessary.

All of these screening tools are free and open for public use.


Anxiety Disorders

 Hamilton Anxiety Scale

Developed in 1959 by Max Hamilton to assess severity of symptoms related to anxiety disorders. 
Attention-Deficit/Hyperactivity Disorder 
(ADHD, also known as ADD) 


Adult ADHD Self-Report Scale

Developed from the diagnostic criteria for ADHD to assess the likelihood of a diagnosis of ADHD.
Bipolar Disorder

Mood Disorder Questionnaire

Developed in 2000 by Dr. Robert M. A. Hirschfeld to assess the likelihood of Bipolar Illness and related disorders.

Depression

 Patient Health Questionnaire-9

Developed from the diagnostic criteria for Major Depressive Disorder to assess the likelihood of a diagnosis of Major Depression and related disorders.
Substance Abuse Disorders
Alcohol Use Disorders Identification Test

Developed by the World Health Organization in 1982 to assess the likelihood of Alcohol Abuse and related disorders.
The Drug Abuse Screening Test (illicit drugs)

Developed in 1982 by Dr. Harvey A. Skinner from The Addiction Research Foundation to screen the likelihood of Substance Abuse and related disorders.
