Online bill payments
You can make payments online for outstanding balances through the online patient portal. The payment system uses Stripe to process all credit/debit card transactions. Log into your patient portal to pay online: portal.kareo.com/app/new/login. If you do not have access to your patient portal, please request access by contacting me directly. If you have received a paper statement, you also can pay online through the link printed at the bottom of the statement page. Telehealth services
Telehealth is the use of video/audio technology to provide real-time health assessment and management, without meeting in person. While in-person visits are the primary and preferred method of treatment, telehealth services will be offered only in certain circumstances (i.e., snowstorm, car troubles) or as mutually agreed upon. Anyone wishing to use telehealth services must complete and return to me this Consent for Telehealth Treatment document For all telehealth services, I use a video product integrated into my Electronic Health Records system, called Kareo. This video service is web-based, and there is no program or app to download. Kareo may be run on your smartphone, tablet, or computer. You will be sent a video link prior to your appointment directly from Kareo. If you have not received the email, please check your Spam folder. You can also access the video link at any time by clicking here: telehealth.kareo.com/jeremysantacroce. At the time of your scheduled appointment, simply click on the link that was sent to you, verify your birthdate, click the "Knock to be let in" button, and I will start the video session when ready. Privacy laws demand that both the provider and the patient must be in private settings where no unauthorized individuals may have access to the session.
